CONTRACT
NEW ENROLLMENT: There is a one-time enrollment fee of fifty dollars. This fee is non-refundable.
TRIAL PERIOD: There is a two week trial period at the beginning of childcare, to make sure that the child and the childcare do not have any conflicts. At anytime during the two week trial, either the parent’s/guardian’s or the provider may terminate care with one days notice. After the trial period of two weeks is over, removing the child applies as in the contract.
HOURS: My child will be attending __MONDAY - FRIDAY__ each week. My child will be dropped off at _________ a.m. and picked up at the latest __________ p.m. If your child is dropped off later or picked up earlier than the times stated, the rate is the same. If your child is dropped off earlier or picked up later than the scheduled times, a fee will be charged.
PAYMENT OF TUITION: The rate is $135.00 per week per child. Payment is due in full whether or not your child is in attendance. Payment’s may be made in cash or check, and payable on FRIDAY BY 5:30 P.M. in advance to childcare services being provided for the next week. All rates are subject to change with a written two weeks notice.
LATE PAYMENT OF TUITION: A charge of $10.00 per day, per child will be collected for payment not posted on Friday P.M., for the following week.
EXTRA HOUR CHARGES: $5.00 per hour, per child.
EXTRA DAY CHARGES: $35.00 per day, per child.
LATE PICK UP CHARGE: Late charges will be collected the same day as services rendered. The late fee charge is $1.00 per every minute late, this must be paid in cash the same day. If you notify the Child Care Facility in advance that you will need additional time, either in the morning or evening, the extra hour charge will apply of $5.00 per every additional hour, per child.
CHECKS: $ 25.00 will be charged for any returned check, plus any fees that are incurred to the child care business. If more than one returned check occurs, then only CASH will be accepted for future payments.
DAMAGES: Parents are responsible for damages to property by their child, such as a broken window, blinds, or anything exceeding $ 25.00 for repair or replacement.
PROFESSIONAL DEVELOPMENT DAYS: When there is a conference or meeting during the week that the provider must attend, to keep furthering my education in child care, the parent’s/guardian’s will be notified in advance. The parent’s/guardian’s will be responsible for finding alternate child care. These days will be paid days for the provider. If these days fall on a Friday, your payment is to be paid on the last open day prior to these days off.
BEREAVEMENT: The provider is allowed one week paid bereavement for immediate family members. The provider is allowed three days paid bereavement for any other family or friends. Parent’s/Guardian’s will be responsible for finding alternate care.
JURY DUTY: If the provider had to attend jury duty selection, these will be paid days. If the provider is selected to sit on a jury, these will be paid days for the provider for the length of the trial. Parent’s/Guardian’s will be responsible for finding alternate care.
BAD WEATHER DAYS: When the Mansfield Independent School District closes due to weather, child care will also be closed. If there is bad weather during the off season, the provider will notify the parent’s/guardian’s by 9:00 p.m. and advise what procedure will be taken for the next day. The provider will inform the parent’s/guardian’s if the child care will be open for the day, open half the day, or closed all day. Due to power outages or severe weather, where the provider is unable to keep a comfortable or controlled environment, child care will be closed. This is also required by Texas DFPS. These days will be paid days for the provider.
TEACHERS & SUMMER CARE: Parent’s/Guardian’s have the option to keep the child enrolled in the childcare during the summer and to pay the weekly tuition, as per the contract/policy. Parent’s/Guardian’s need to call ahead to let the provider know which days the child will be in attendance. (This will help in the preparation of meals, projects & any activities planned for the day).This option will keep a secured spot for the child when full time care/school resumes. This option is also beneficial to keep the child in tune with the childcare and the other children in the childcare. If the parent’s/guardian’s do not wish to pay for summer care, they can remove the child from care. But the child’s spot will be filled and not be open when school restarts.
VACATIONS FOR CHILD CARE CHILDREN: Must be enrolled for six months, before being eligible for vacation. Any vacation time must be submitted to the provider, in writing on a vacation form, two weeks in advance. Child’s one week of unpaid vacation cannot be used towards their payment of the two-week notice to end childcare. Child’s one week of unpaid vacation CANNOT be taken at the same time as the provider’s scheduled paid vacation. Child’s one week of unpaid vacation CANNOT be taken during a holiday week, which the holiday’s are to be paid. The one week of vacation must be taken during a regular scheduled week of child care. Each child will receive one week of vacation per year at no charge. If two weeks is not given, in writing, a charge of full price will be collected. If parent’s/guardian’s are unwilling to pay full price, the child’s slot will be opened for another child and their spot will be terminated immediately. Any more vacation days taken will be at full price.
VACATIONS FOR CHILD CARE PROVIDER: Parent’s/Guardian’s will be notified two weeks in advance. Parent’s/Guardian’s are responsible for finding alternate care and will be charged full price for the providers vacation time. The provider is allotted two paid weeks of vacation per year. Payable before going on vacation.
HOLIDAY’S: The following days are considered paid holiday’s; full payment is due for weeks that include holiday’s.

NOTE: If a holiday falls on a weekend, then I will observe the holiday either on the Friday before or the following Monday.
TERMINATION OF CONTRACT: The Parent’s/ Guardian’s: May terminate this contract by giving a two week written notice. Tuition for the two week written notice of termination is due, when written notice is given. When removing child from child care, all tuition must be current, including the payment of the two week notice tuition.
The Provider: May terminate this contract by giving a two week written notice. Tuition for the two week written notice of termination is due, when the written notice is given to the parent’s/guardian’s. When removing child from child care, all tuition must be current, including the payment of the two week notice tuition.
The Provider: May immediately terminate this contract without giving notice, if the parent’s/guardian’s do not make payments when due, do not pick up an ill child when notified to do so, or the child is a risk to the other children in care, disrupts the daily running of the child care center, is habitually late picking up child-without notification by 12:00 noon daily, or the provider sees any other need to do so.
CHANGE OF CONTRACT: I, the provider, reserve the right to make additions or deletions to this contract at any time. I will give you ten days notice if I exercise this right, and require that you sign the change. It will then be as binding as the original contract.
